Thank you for your interest in the Foundation for Integrative Oncology’s scholarship program.  The PDF form may be filled out on-line and emailed to: info@ComplementaryCancerCare.org or downloaded and mailed to: P.O. Box 5031/Larkspur, CA 94977.  Please note that it is vital that the form be completed in its entirety, as applications that are not filled out completely will be returned.  

 
The goal of the Foundation for Integrative Oncology is to enable people with cancer and their loved ones to have access to and learn about a variety of complementary therapeutic approaches during their state-of-the-art cancer care, and to support research into the effectiveness of these approaches.  The Foundation seeks to assure that its services are available to financially limited patients.  

Because we strive to maximize use of this fund by eligible patients, scholarships are awarded based on financial need and severity of symptoms patients are experiencing.  Priority is generally given to first time applicants. Up to 8 integrative oncology sessions may be granted.  In order to maximize use of this fund by eligible patients, we request that participants pay a portion of the cost for each session. Please check the amount you are able to contribute per session on the application.
